1. How to apply for Health Code or Health Declaration Form?

1.1 The Chinese nationals (including the residents of Hong Kong SAR, Macao SAR and Taiwan) can apply for the Health QR Code through the Wechat mini-program, which can be accessed by scanning the QR code below.

1.2 New Zealand citizens and other foreign nationals can log on the following website to apply for the Health Declaration Forms. https://hrhk.cs.mfa.gov.cn/H5/

For the sake of convenience, both Health Code and Health Declaration Form are referred to as health code hereinafter.

  

2. What documents are required for health code application?

2.1 Required documents prior to travel (please only take the test after 14 days you have been fully vaccinated for COVID-19, exclude booster dose):

1) For people who have fully vaccinated with inactivated COVID-19 vaccine, please provide two negative COVID-19 PCR test reports;

2) For people who have fully vaccinated with mRNA COVID-19 vaccine, please provide a negative COVID PCR test report and a negative IgM (Nucleocapsid) serology antibody test report;

3) For people who have not been vaccinated, or not been fully vaccinated against COVID-19, please provide a negative COVID PCR test report and a negative IgM (Nucleocapsid) serology antibody test report;

2.2 Vaccination Certificate

Being fully vaccinated means that people have received all required doses of inactivated vaccine or mRNA vaccine within the recommended gap between doses, and the last dose being at least 14 days. The most common inactivated vaccines are Sinopharm (2 doses required) and Sinovac (2 doses required), and mRNA vaccines are Pfizer (2 doses required), Moderna (2 doses required), and AstraZeneca (2 doses required).

1) People who have been vaccinated in China, please provide the International Travel Health Certificate which can be obtained through the COVID-19 Prevention Health Code International version mini program on Wechat (refer to the pictures below). In case of the certificate could not be obtained through the Wechat app, please provide other forms of the vaccination certificate issued by a Chinese government official, and complete the Letter of Commitment on COVID-19 Vaccination (refer to Appendix 1, Chinese nationals must sign the letter with Chinese characters).

                  

2) People who have been vaccinated in New Zealand, please provide the International Travel Vaccination Certificate (refer to the pictures below) which can be obtained from the website of New Zealand Ministry of Health. Alternatively, people may request an immunization status from their GP with the following information: full name, date of birth, name of vaccine, date of the first and second doses were given, and place of given. It also must be signed and stamped by GP or clinic. The Vaccine Pass to be used domestically and the hand written COVID-19 Vaccine Record Card are not acceptable as proof of vaccination.

               

3) People who have not been vaccinated are not required to provide such a certificate.


2.3 Proof of residence

1) Chinese nationals (including the residents of Hong Kong SAR, Macau SAR and Taiwan) should upload their New Zealand visas or residence permits. 

2) New Zealand citizens should upload the information pages of their valid passports and valid Chinese visas issued after 28 March 2020, or valid or Chinese residence permit.

3) Nationals of the third countries should upload the information pages of their valid passports, and valid Chinese visas issued after 28 March 2020, or valid Chinese residence permit, and valid New Zealand visa or residence permit.

2.4 Travel itinerary

Electronic ticket or flight itinerary issued by airline company or travel agent bearing full name of the passenger and detailed flight information. The ticket must show the origin of departure is from New Zealand to China. Flight itinerary without personal information and flight details, and screenshot of flight booking from text message, are not acceptable.

2.5 Personal Health Monitoring Prior to Travel

Please start to monitor your personal health 8 days prior to travel by completing the Declaration of Commitment on COVID-19 Close-Loop Management and Personal Health Monitoring (refer to the Appendix 5). Please ensure you have a negative Rapid antigen test (RAT) result before initiate the procedure of self-monitoring, the starting date is 8 days prior to board your flight, and to record your body temperature daily until the day before your flight (for example, passengers flying to China on 14 March, should start the self-monitoring from 7 March until 13 March). 

You do not have to upload the form when applying for the health code, however, print it out and sign (for a minor, it must be signed by an accompanying legal guardian), hand it to the airliners for check and for future reference when boarding. You may be declined to board the flight if you failed to cooperate or not meet the requirement. 

3. How to choose the date for the tests?

The validity of health code is counted on the basis of "sampling date +2 days". 

3.1 People who have received inactivated vaccines, please have two separate COVID-19 PCR tests within two days prior to board the China-bound flight. The two COVID-19 PCR tests must be conducted by different laboratories. If the two tests have been conducted in two different cities, please request for the health code from the Chinese diplomatic and consular missions in accordance with the consular jurisdiction based on the location where the first test has been conducted. In case of such requirement cannot be met by the laboratories at your location, you have the option to have two separate COVID-19 PCR tests within three days prior to board the China-bound flight, however, the two tests must be done with a sampling interval of over 24 hours, and the sampling for the second test must be taken within 48 hours before boarding. People in this category who have completed two COVID-19 PCR tests are not required to have an IgM antibody test.

3.2 People who have received mRNA vaccines, please have one COVID-19 PCR test and one IgM Nucleocapsid antibody test done 2 days before boarding on China-bound flight. If the sampling date is not available on the test reports, the earliest date as shown on the test reports will be used as the sampling date. For example, if a flight departs on 3 June, then the earliest date to go for the tests is 1 June.

3.3 People who have not been vaccinated or not been fully vaccinated, please follow the requirement as people who have received mRNA vaccines.

4. What are the requirements on the test reports?

The Chinese diplomatic and consular missions in New Zealand only accept the report on the nucleic acid test conducted with a nose or throat swab specimen (Nasopharyngeal Swab), and COVID IgM (Nucleocapsid) serology antibody with venepuncture blood sampling. The report must show negative test result if a total serology antibody test (IgM + IgG) has been tested.

If he/she applies for the health code, he/she should submit the originals of negative COVID PCR and IgM (Nucleocapsid) serology antibody test reports issued by a test facility. The reports must include full name (must match the full name shown on your passport or travel document), date of birth, and date of tests (sampling), and test results, and name of the testing facility. Reports issued by a GP or a clinic (including Inbox reports) must distinctly state the name of the testing facility, and be signed by the GP or carry the stamp of the clinic.

Reports will not be accepted if any required information is missing or without the above-mentioned signature or stamp. Nor will text messages indicating test results be accepted.

5. What are the acceptable travel documents?

When applying for the health code, New Zealand citizens or citizens from third country, should submit valid passport. Mainland Chinese citizen should submit valid Chinese passport. Residents of Hong Kong SAR, Macao SAR and Taiwan should submit the corresponding Mainland Travel Permit for Hong Kong and Macao Residents, or Mainland Travel Permit for Taiwan Residents, or Chinese Travel Document.

Please make sure that the number of travel document you provided matches the number shown on your test report, vaccination certificate, proof of residence, and travel itinerary.

6. What if I do not have a valid proof of NZ residence?

6.1 If your New Zealand visa or residence permit has expired less than one year, please provide a copy of the expired visa and complete the declaration of overstaying in New Zealand (refer to Appendix 4).

6.2 If your New Zealand visa or residence permit has expired more than one year, please provide one of the following documents and complete the declaration of overstaying in New Zealand (refer to Appendix 4):

Legislative letter or evidence issued by Immigration New Zealand;

Utility bills bearing full name issued by power, water or internet provider for over three months;

Other documents carrying personal information and reaffirming long term residence in New Zealand.

7. What if I have received mixed COVID-19 vaccines?

We consider the situation based on whether you have been fully vaccinated by inactivated vaccine or mRNA vaccines, both types of vaccine are required two doses.

7.1 You have been fully vaccinated by inactivated vaccine, and have received additional Pfizer vaccine, then you are considered as vaccinated with inactivated vaccine.

7.2 You have been fully vaccinated by Pfizer, and have received an additional inactivated vaccine, then you are considered as vaccinated with mRNA vaccine.

7.3 You have been fully vaccinated with inactivated vaccine and Pfizer vaccine, then you are considered as vaccinated with inactivated vaccine.

7.4 You have received one dose of inactivated vaccine and one dose of Pfizer vaccine, then you are considered as unvaccinated.

7.5 You have received one dose of Sinopharm vaccine and on dose of Sinovac, you are considered as vaccinated with inactivated vaccine.

8. Does he/she need to take the COVID PCR test and the IgM serology antibody if vaccinated against COVID-19?

Even after getting a COVID-19 vaccine, he/she is required to take the two COVID-19 PCR tests or a COVID-19 PCR test and the IgM serology antibody test, and should also upload the vaccination certificate for COVID-19 regardless of the test results.

9. If fully vaccinated, does he/she still need to be quarantined upon arrival?

At the moment, all travelers arriving in China are subject to managed quarantine and relevant tests. The Disease Control and Prevention Office at the port of entry has the final interpretation of the quarantine requirements and types of tests.

10. When should the application for health code be uploaded?

He/she is advised to submit the application through a proper platform upon receiving the test reports and pay close attention to the outcome of the application. If the application has been declined (shown in red color), please provide information as shown on the notification and make the request again.

Please have all required documents ready and apply for the health code as soon as possible. If the flights depart after midnight or in the morning, please submit application no later than 8pm on the previous day. For flights depart on other times, please submit application no less than 5 hours before the scheduled departure time.

All applications for the health code will be reviewed manually, and may take a longer time to process. Please check your status of your request through the app or website, and only contact us in case of emergency.


11. Do you have any tips for submission of personal information?

11.1 Applicants who are the Chinese citizens (including the residents of Hong Kong SAR, Macao SAR and Taiwan) should enter their full names in Chinese as shown on travel documents as well as the travel document numbers. The residents of Hong Kong SAR, Macao SAR and Taiwan should also upload a picture of the travel document.

11.2 Please provide your New Zealand local mobile phone number, or if you don’t have a local phone number, you have provided your Chinese mobile phone number, please make sure the number you provided is reachable, in case we need to talk to you.

11.3 Please submit the application for the health code to the mission whose consular service covers the location of the testing facility, as shown below:

Chinese Embassy in New Zealand: Wellington, Manawatu-Wanganui, Hawke's Bay, Taranaki, Gisborne, Bay of Plenty

Chinese Consulate-General in Auckland: Auckland, Northland, Waikato

Chinese Consulate-General in Christchurch: South Island

12. What could he/she do if he/she made a mistake in submitting the materials for the health code, or the application were declined?

He/she will be notified of the reasons for any failure of application through the Wechat mini program or H5 website. He/she is requested to re-submit the application for the health code after making due adjustments as required.


13. Does he/she need to bring the hard copies of test reports when boarding flight to China after receiving the green health codes?

He/she is recommended to bring the hard copies of the negative test reports and vaccination certificates in case of inspection. He/she may be declined to board if he/she refused to cooperate with airline when verifying documents.

14. Do infants have to apply for the health code?

Children under the age of 5 years old (on the day of departure) are required to apply for the health code by providing one negative PCR test report two days before boarding flights to China with other required documents, provided that an accompanying legal guardian has received a valid health code for the China-bound flight from New Zealand.

15. How does a seafarer apply for health code?

Seafarer who disembarks in New Zealand and departing for China from Auckland need to take COVID-19 PCR test after disembarkation and get a negative test result, and then go into a 14 days self-isolation and closed-loop management. He/She can apply for the health code 48 hours prior to board China-bound flight by providing the Seafarer Closed-loop Management Commitment Letter from his/her employer (refer to Appendix 3), and negative test results of COVID-19 PCR, and IgM (Nucleocapsid) antibody after completing the self-isolation. It is important that all relevant parties, including the seafarers, the shipping company, and agent of the shipping company, to get information on entering New Zealand and its current quarantine requirement. Please consider carefully to disembark in New Zealand if the requirement cannot be met.

16. People who had been infected with COVID-19.

If he/she has been diagnosed with COVID-19, or has not been confirmed case of COVID-19, however received positive test reports of any of the following tests: COVID-19 PCR, IgM and IgG antibody, are considered as infected with COVID-19(unless as a result of COVID-19 vaccination), and he/she can only apply for the health code after fully recovered, and completed a period in isolation and health monitoring and relevant health check. The procedure is as followed:

(1) After he/she has fully recovered, he/she must provide a diagnosis evidence of CT lung scanning or X-Ray from a qualified health provider, excluding pregnant women, confirming that he/she has not been infected with COVID-19 or had been infected with COVID-19 but has since fully recovered. He/she must also complete two COVID-19 PCR tests showing negative test results (samples must be taken at 24 hours interval), and also send test reports for verification within 3 days to the Chinese Embassy or Consulate-General in accordance with consular district jurisdiction.

(2) He/she must go into self-isolation and monitor health for 14 days once the verification has been completed, and also complete the Personal Health Monitoring Form (refer to Appendix 2).

(3) The Chinese Embassy or Consulate-General in New Zealand confirms that he/she meets the requirement, he/she can request for the health code (health declaration from) by providing two PCR tests within 48 hours prior to board China-bound flight. (Please refer to Section 3)

Please contact the Chinese Embassy or Consulate-General in New Zealand in advance to avoid travel delays.

17. Close contact or people showing suspect symptom

He/She should go into a 14 days self-isolation immediately, monitor health, and complete the Personal Health Monitoring Form (refer to Appendix 2). Please take COVID-19 PCR tests on day 1, day 4, and day 7 during the monitoring period, if all tests came back with negative test results, he/she can apply for the health code (health declaration from) within 48 hours prior to board China-bound flight by providing negative test results of COVID-19 PCRtest and IgM (Nucleocapsid)antibody test after completing the self-isolation. (Refer to Section 3) 

18. Could he/she fly to China via a third country?

Given the current situation of COVID-19 pandemic across the world, international travelling is exposed to high risk of cross-infection of the coronavirus. In order to avoid getting infected during transiting through other countries, he/she is requested to choose a non-stop flight from New Zealand to China, instead of travelling to China via a third country. Travelers from countries with direct flights to China must not transit through New Zealand.
